Photo Request Form

Thank you for your interest in College of Engineering photography services! Please fill out the form below and press submit. Your submission will be routed to the communications team, and you will receive an initial response within two business days.

This form is for College of Engineering faculty, administrators/staff, and students only. If you are external to the College and are looking for photo-related assistance, please email coephoto@uic.edu instead.

A few notes before you begin:

  • We may not always be able to fulfill all requests, but we will do our best, and we will give each request careful consideration.
  • If you have a question about College photography that cannot be addressed via the form below, please email coephoto@uic.edu for assistance.

Photography Request Form

  • Please provide the number we can reach you on the day of a photo shoot (e.g., cell phone, if applicable).
  • Please select from the drop-down menu below. If you are representing a College of Engineering student group, please choose "COE student organization" from this drop-down and include the name of your org in the description field below.
  • Please use this box ONLY for special events that take place on a specific date and at a specific time. If this does not apply to your request, leave this box blank.
  • Please leave this field blank if your request is open-ended or flexible.
